Print a copy for your use and send an electronic copy to proctor@ccdmin.org NOTE- Please print and distribute three (or more) Minister/Student Evaluation Forms to individuals that know you or of your work, such as 1)_denominational rep., 2) ordination committee liaison, 3) home church pastor, 4) seminary professor.

You will receive information about how to access online websites for taking the following inventories after you have registered and sent your word documents to proctor@ccdmin.org

The Strong Interest Inventory (25 minutes)

Minnesota Multiphasic Personality Inventory - 2 (administered in November for the January group by Rev. West)
